Pre- and Post-Operative Orders: Bladder Tumor Resection


Pre-Op Bladder Tumor Resection (TURBT) Orders

Dx: Bladder tumor

Allergies:

Labs:

       CBC
       Basic Metabolic Panel (Chem 9)

Xray: Chest Xray if needed by anesthesia or hospital policy

Cardiology: EKG, if needed by anesthesia or hospital policy

Diet: NPO after midnight except for AM medicines with a sip of water

Patient instructions: Hold aspirin, Plavix for one week prior to surgery

IV: per anesthesia (Ringer's Lactate at 100cc/hr if no anesthesia preference)

Consent: Transurethral resection of bladder tumor, bladder and urethral biopsy, possible ureteral wash/retrograde pyelogram

Shave: None

Meds: Cefazolin (eg. Ancef) 1 mg IV in pre-op area

Post-Op Bladder Tumor Resection (TURBT) Orders

Dx: Bladder tumor

Allergies:

Diet: Regular diet as tolerated.

IV: Current IV at 125cc/hr.

VS and output per recovery guidelines.

Irrigate Foley with Normal Saline prn clots or failure to drain

Meds:
   B&O suppository 30-60 q6h prn bladder spasms.
   Hydrocodone 5mg with acetaminophen 500mg, 1-2 q6h prn.
   Morphine 2-4mg IV q 4 hours prn severe pain.
   Cefazolin (eg. Ancef) 1 mg IV q 8h.

Discharge when alert and urine clear (rose' is OK).
If discharged with Foley in place please send home with the irrigating syringe, saline, and instructions for use, as well as instructions on catheter care and removal (cut off the balloon port, never "pull it out")
